So a very mentally ill man and another male who was dressed as a woman. She played a kind of parent to the ill man, babied him loads. They break into a house and kill the mother and tried to take her two daughters,  although the film for ages makes out as if the daughters escaped. This turned out to be an escape mechanism in the form of dissociation. The mentally ill guy is using the girls and dressing them up as dolls, torturing them etc. The girls eventually escape and they are running through a field towards a cop car, they think they are saved and suddenly an ice cream truck pulls up. The two psychopaths hop out of the truck and shoot the cop and drag them back into captivity. They endure some more torture and actually finally escape
